Avaliação do Efeito da Pandemia de Coronavírus na Demanda de Encaminhamentos aos Serviços de Neurologia e Neurocirurgia

Assessment of Coronavirus Pandemic Effect on Demand for Referrals to Neurology and Neurosurgery Services

Resumo

Introdução: A pandemia de coronavírus resultou em importante impacto nas relações entre as populações, não só em termos de saúde, como também de ordem econômica, social e mesmo geopolítica. Estima-se que dentre os pacientes que cursam com comprometimento pulmonar severo durante a vigência da doença, 45% deles irão apresentar sintomas neurológicos associados. Objetivo: O presente estudo teve por objetivo mensurar o impacto que o coronavírus provocou na demanda de encaminhamentos aos serviços de atenção especializada em Neurologia e Neurocirurgia. Métodos: Foram recuperadas informações constantes em uma base de dados própria do município (IPM), onde constam solicitações de referência-contrarreferência realizadas durante o período objeto desse estudo. Utilizou-se a planilha de dados do programa Microsoft Excel 365 para organização, coleta, contagem e cálculos relacionados às variáveis a serem determinadas. Resultados e discussão: Observou-se equilíbrio na procura pela maioria dos serviços nas estratificações de gênero e faixa etária dos pacientes. A partir da comparação na procura aos serviços no período pré-pandêmico e pandêmico, os cálculos demonstraram redução absoluta de encaminhamentos durante a pandemia. Na avaliação das diferenças relativas de procura entre as especialidades, comparando- se especialidades cirúrgicas, a neurocirurgia demonstrou aumento relativo com variação estatisticamente significativa. O mesmo efeito não foi observado a partir da análise da proporção de pacientes que foram encaminhados a neurologia em relação às outras especialidades clínicas. Conclusão: Outros trabalhos deverão ser conduzidos para melhor entender a correlação de efeitos da infecção por SARS-COV-2 na demanda aos serviços de neurocirurgia na pandemia, e a ausência de efeito nos encaminhamentos ao serviço de neurologia.

Abstract

Introduction: The Coronavirus pandemic has resulted in an important impact on relations between populations, not only in terms of health, but also in the economic, social and even geopolitical order. It is estimated that among patients with severe pulmonary involvement during the disease, 45% will present associated neurological symptoms. Objective: to measure the impact of coronavirus for referrals to special care services in Neurology and Neurosurgery. Method: Information was retrieved from the municipality’s database (IPM), when available the reference/counter-reference requests during the period of this study. Microsoft Excel 365 spreadsheets were used to organize, collect, count, and calculate data pertaining to the variables to be determined. Results and discussion: There was a balance in most services facing stratifications of patients’ gender and age group. By comparing pre-pandemic and pandemic periods, the figures revealed an absolute lowering in referrals during the later one. Assessing the relative differences among specialties, comparing the surgical ones, neurosurgery showed a relative increase and statistically significant variation. The same was not observed upon analysis of the proportion of patients who were referred to neurology, in comparison to other clinical specialties. Despite the monthly fluctuations, the global rate was similar to referrals to other clinical specialties. Conclusion: Further studies must be carried out in order to have a better understanding of the relationship between the effect of SARS-COV-2 infection on the search for neurosurgical services, as well as the the lack of effect on referrals to the clinical specialty of neurology.
